This position is ideal for a motivated professional who enjoys working with elderly and disabled individuals in the community while helping coordinate quality care and support services.
Responsibilities:
Conduct initial and ongoing member assessments Develop and update individualized care plans Perform routine member visits and wellness checks Coordinate services with caregivers, families, providers, and community resources Maintain accurate and timely documentation in compliance with MassHealth regulations Assist members with accessing benefits and support services Communicate changes in condition or concerns to nursing/clinical staff Ensure compliance with
GAFC/AFC
program standards
Qualifications:
Experience in case management, care coordination, social services, home care, or healthcare preferred Background in GAFC, AFC, home health, or elder services strongly preferred Excellent communication and organizational skills Ability to work independently and manage schedule efficiently Reliable transportation required for community visits Bilingual candidates encouraged to apply
Position Type:
Per-Diem / Flexible Schedule Competitive per-visit or hourly compensation based on experience
