International Health Services, Inc. International Health Services, Inc. is seeking a compassionate and organized GAFC Care Manager to join our team. This role is essential in ensuring our Group Adult Foster Care (GAFC) members receive high-quality, coordinated services that meet their individual needs.

Key Responsibilities:

Conduct initial and ongoing assessments of GAFC members to determine care needs. Develop and maintain individualized care plans. Coordinate services and referrals to community resources. Maintain accurate and timely documentation in compliance with program and state regulations. Collaborate with interdisciplinary team members, caregivers, and providers to support member well-being. Communicate effectively with members and families in both English and Spanish .

Qualifications:

Bachelor's Degree in Social Work, Human Services, Psychology, Nursing, or related field (required). Experience working with elderly and/or disabled populations preferred. Strong organizational and communication skills. Ability to work independently and as part of a team. Valid driver's license and reliable transportation preferred.
