About the Position We are seeking a compassionate, dependable, and highly organized Full-Time Care Manager to join our growing team. This is a community-based position involving assessments, placements, ongoing home visits, care coordination, documentation, and communication with individuals, caregivers, and members of the interdisciplinary team. The position requires regular travel throughout an assigned geographic territory in Massachusetts. Schedule Monday through Friday 8:30 AM to 4:30 PM Qualifications Bachelor's degree in a related human services, healthcare, social services, or behavioral health field. Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area. Ability to successfully complete all required background and credentialing checks. Strong communication, organizational, documentation, and time-management skills. Ability to work independently in a community-based environment while maintaining effective communication with the interdisciplinary team. Experience At least two years of relevant experience working with older adults or individuals with disabilities in a healthcare, community-based, human services, social services, or related setting; or A current social work license issued by the Massachusetts Board of Registration of Social Workers, as permitted by applicable program requirements.
