The Care Manager is responsible for delivering relationship-centered, early intervention services to youth and families enrolled in the C.A.R.E. program. This role focuses on consistent engagement, individualized care planning, advocacy, and coordination of services across family, school, and community systems. The Care Manager serves as the primary point of contact for youth and caregivers, providing weekly, in-person support, ongoing assessment, and connection to services that promote stability, growth, and long-term success.Essential Functions and Responsibilities:
1. Relationship-Based Case Management Manage a caseload of youth (ages 10-17) enrolled in the C.A.R.E. program Conduct weekly, in-person meetings with youth (including required make-up sessions) Build strong, trust-based relationships with youth and caregivers 2. Intake, Assessment, and Care Planning Conduct intake and comprehensive assessments to identify needs and strengths Develop and implement individualized care plans Monitor progress and adjust plans through ongoing assessment 3. Counseling, Mentorship, and Skill Development Provide and/or coordinate: Individual and group counseling Mentoring and life skills development Restorative practices and social-emotional support 4. Family Engagement and Support Engage parents/caregivers through: Parent education sessions Regular communication and support Strengthen family stability and involvement in youth progress 5. Advocacy and Systems Navigation Advocate for youth across systems (school, healthcare, social services, etc.) Identify and address barriers such as: Academic challenges Behavioral concerns Access to services (transportation, mental health, etc.) 6. Service Coordination & Program Integration Connect youth to supportive services including: Tutoring and academic enrichment Afterschool programming Summer day camp and enrichment opportunities Collaborate with schools, community partners, and internal Yunion programs 7. Documentation and Reporting Maintain accurate, timely, and confidential records of: Assessments Care plans Progress notes and outcomes Track participation, engagement, and impact metricsMinimum Qualifications:
Bachelor's degree in Social Work, Psychology, Counseling, or related field (Master's preferred) Minimum 2 years experience working with youth and families in: Early intervention Youth development Social services or behavioral health Experience working with at-risk or system-involved youth Strong skills in case management, relationship-building, and coordinationPreferred Qualifications:
Master's degree in Social Work, Counseling, Public Health, or related field Training in: o Trauma-informed care o Restorative practices o Crisis interventionCore Competencies & Attributes Relationship-Centered Approach:
