$75,000-$85,000 per year About the Role We are seeking an experienced Palliative Care Manager (LMSW) to help launch and lead a brand-new palliative care program serving Grand Rapids and surrounding communities. This is a unique leadership opportunity for a social worker who wants to make a meaningful impact while helping build a program from the ground up. Reporting directly to the CEO, the Palliative Care Manager will play a key role in developing workflows, establishing operational processes, supporting staff, ensuring regulatory compliance, and driving program growth. Key Responsibilities Lead all palliative care program startup activities, including workflows, policies, and operational design Manage program budget and financial performance Supervise and support palliative care staff in collaboration with the Medical Director Ensure compliance with Medicare, Medicaid, commercial payers, and accreditation standards Support provider credentialing and payer contracting Oversee scheduling, referrals, documentation, and care transitions Drive program growth, customer service, and performance improvement Provide social work services during program startup and as needed Participate in the Manager On-Call rotation for hospice operations Collaborate with interdisciplinary teams to improve patient and family outcomes Qualifications Master's degree in Social Work (MSW) required LMSW credential required Hospice or palliative care leadership experience preferred Prior program development experience preferred Experience working with Electronic Medical Records (EMR) Proficiency with Microsoft Office products Strong leadership, communication, and interpersonal skills Ability to collaborate effectively across disciplines Demonstrated ability to lead teams through growth and change Compensation & Benefits $75,000-$85,000 annual salary Mileage reimbursement at the federal rate Generous paid time off (PTO) 403(b) retirement plan with employer match Medical, dental, and vision insurance starting Day One Employer-paid life insurance Employer-paid short-term disability Self-care coaching Employee Assistance Program (EAP) Company-provided technology as needed, including laptop/iPad and cell phone Work Schedule Monday-Friday | 8:30 AM-5:00 PM Participation in a manager on-call rotation for hospice operations may be required.
