Care Manager - Charlotte, NC On-site | Full-Time | On-Call Rotation Required Are you passionate about helping others live healthier, more independent lives? Community Choices is looking for a dedicated Care Manager to join our team in Charlotte, NC. In this rewarding role, you'll make a direct impact by guiding individuals through life transitions, connecting them to resources, and creating personalized care plans that support whole-person wellness. What You'll Do Build and manage person-centered care plans through assessments and ongoing support Serve as the main connection between members, providers, caregivers, and community partners Help members navigate transitions such as hospital discharge or reentry into the community Lead care team meetings and coordinate services across multiple providers Advocate for members while promoting confidence and independence Maintain accurate documentation and case notes Participate in an on-call rotation and provide transportation support when needed What You Bring Bachelor's degree in psychology, sociology, social work, nursing, or related field At least 2 years of experience supporting individuals with behavioral health disorders Strong communication, organization, and problem-solving skills Passion for person-centered care and working in the community Preferred Experience Care management or case management background Experience supporting chronic physical or behavioral health needs Bilingual or multilingual skills Requirements Valid driver's license, clean driving record, and auto insurance Ability to travel locally and occasionally lift up to 50 lbs Ability to maintain CPR/First Aid and other required certifications Why Join Community Choices?
