Job Description

SUMMARY:
The Independent Living Program (ILP) Case Manager serves as Case Manager for ILP residents. This person will establish supportive, professional relationships with program staff, residents, stakeholders, and families; ensuring quality of care, program implementation, regulatory compliance, and household expectations. This person advises residents in assessing individual needs and setting goals/monitoring progress and advocates for residents' access to community resources and opportunities in support of the agency's vision.
DUTIES AND RESPONSIBILITIES
1. Ensure the ILP cottage is a safe and secure environment for residents in accordance with all regulatory standards through visual observation, inspection, documentation and correction. 2. Serve as the main point of contact for residents for guidance in handling on-going issues within the cottage, new or unusual circumstances or events, and instruction during emergency situations. 3. Provide on-call support after hours on nights, weekends, and holidays to assist residents over the phone and in-person when needed. 4. Participate in staff meetings, in-services, case reviews, PQI and other meetings as scheduled. 5. Ensure all serious incidents within the ILP program are verbally communicated to the Vice President of Operations and documented in ExtendedReach in accordance with ACH policy and procedure. 6. Provide orientation training for ILP residents and ensure ACH policies and procedures are understood and implemented. 7. Complete monthly documentation to ensure residents have completed goals for the month to earn their monthly stipend. 8. Communicate with all ACH staff in a manner that supports a team approach; speak respectfully to everyone and never resort to yelling or using profanity. 9. Demonstrate knowledge and application of the CARE model and TCI system in your daily practice as a human services professional. 10. Educate and mentor residents in the utilization of CARE and TCI strategies to minimize interpersonal conflict and maintain consistent structure in the cottages. 11. Inspect cottages for proper facility maintenance/upkeep, acceptable inventory levels of all necessary household supplies, and correct or report issues when needed. 12. Assist with the coordination of ILP transportation needs and provide resident transport in an agency vehicle when needed. 13. Collaborate with DSS Social Workers, guardian's ad litem, and other professionals or vested parties to promote a holistic team approach in the service of residents. 14. Lead the intake process for ILP through collaboration with the ACH Independent Living Team to review and select candidates for admission. 15. Secure all required information, documentation, and signatures upon acceptance into the program and at the time of intake/discharge. Responsible for measuring outcomes, record keeping/review, and maintenance of all case files with up-to-date information as specified by state licensing and accreditation standards. 16. Ensure access to counseling and appropriate services for residents and monitor schedule of health care appointments to comply with licensing and accreditation standards. 17. Assist residents with self-assessment of strengths and needs to create and/or update the individual service plan, set goals, and evaluate progress. 18. Cultivate caring, supportive relationships with residents and provide a minimum of weekly individual face-to-face time with each resident. 19. Participate in ongoing training and development opportunities to enhance your professional skill set and meet training hour requirements for your position. 20. Attend benefits, fund-raising events, group outings, and other activities or events that occasionally occur outside of scheduled working hours when required by the agency or department. 21. Perform other duties as assigned by the VP of Operations and CEO/President.
QUALIFICATIONS
  • Bachelor's degree in social work is preferred; a bachelor's degree in a human services field and 2 years of related experience or an equivalent combination of education and experience may be accepted based on NCDHHS guidelines
  • Valid driver's license and acceptable safe driving record must be maintained
  • Intermediate computer skills
  • CARE and TCI certification (or willing to become certified through the agency)
  • 1st Aid, CPR certification (or willing to become certified through the agency)
  • Experience working with young adults is preferred
