Job Description

Job Summary We are seeking a dedicated and compassionate Care Manager to join our healthcare team. The Care Manager will play a pivotal role in coordinating patient care, ensuring that individuals receive the necessary support and resources throughout their healthcare journey. This position requires strong communication and supervisory skills, plan development, and knowledge of the behavioral health system. Duties Assess patient needs through diagnostic evaluations to develop and review individualized care plans. Coordinate with Care Managers, Extenders, primary care providers and specialists to ensure continuity of care across various settings, including acute care and critical care environments. Facilitate communication between patients, families, and healthcare teams to promote understanding of treatment options and health management strategies. Monitor patient progress and adjust care plans as necessary based on ongoing evaluations and managed care protocols. Review documentation entered by Care Managers and Extenders in Clinically Integrated Network system. Communicate with Managed Care Organizations (MCO). Document supervision with Care Managers and Extenders. Participate in clinic activities aimed at improving patient outcomes and enhancing service delivery. Experience Licensed as
LCSW, LCMHC
Dual license is preferred A background in behavioral health is highly desirable. Knowledge of Tailored Care Management. Familiarity with primary care practices, acute care settings, and critical care environments is essential. Previous experience in managed care settings will be an advantage. Strong interpersonal skills are necessary for building relationships with patients and their families while providing empathetic support. $75,000.00 - $80,000.00 per year
