Full Job Description Nursing Home Admissions Coordinator Job Summary:
We are seeking a highly organized and detail-oriented Nursing Home Admissions Coordinator to join our team. As a key member of our admissions team, you will be responsible for ensuring a smooth transition for new patients and their families. If you have a prior experience as a nursing home admissions coordinator, and a strong attention to detail, we encourage you to apply for this full-time opportunity.
Nursing Home Admissions Coordinator Responsibilities:
Coordinate the admissions process for new patients, including obtaining necessary documentation and verifying insurance information Develop and maintain strong relationships with referral sources, including physicians, hospitals, and other healthcare providers Ensure that all necessary paperwork and medical records are completed and accurate, assisting with any discrepancies or issues that may arise Collaborate with the care team to develop personalized care plans for each new patient Provide exceptional customer service to patients and their families, ensuring a positive experience from arrival to discharge Maintain accurate and up-to-date records, reports, and statistical data Participate in quality improvement initiatives to enhance the overall patient experience
