  • Hourly rate $43.00
  • The areas highest quality behavioral healthcare providing care management services to clients which includes assessing and assisting with their needs and coordinating care in a warm and compassionate environment, 8 +
  • hours per week, 9:00am
  • 5:00pm Monday
  • Friday
  • The Care Manager will follow through and assist clients with their plan of care, and coordinate care services ensuring that those we serve receive the highest quality of service in a caring and compassionate atmosphere.

Bachelor's degree is required Ability to read, write, understand, and communicate effectively in English Drivers license in good standing Possess insured vehicle to travel to and from clients homes Ability to deal with others in a diplomatic, tactful and confidential manner Ability to maintain composure under stress or in dealing with residents, staff, and families Flu vaccine required during flu season
