Integrity House operates 18 licensed treatment programs across New Jersey as well as community outreach and prevention programs and serves over 3,000 individuals every year. We are excited to announce our Care Manager opening in Newark, NJ ! The Care Manager operates within our Certified Community Behavioral Health Clinic and is responsible for empowering individuals by providing direct support services, including psychoeducation, advocacy, crisis intervention, and skill development. The Care Manager coordinates care with agency, community providers, and healthcare facilities to ensure comprehensive service delivery and conducts holistic care management assessments addressing key life domains such as housing, finances, healthcare, and employment. This position is responsible for collaborating within an interdisciplinary team to address social determinants of health and link clients to essential medical and treatment resources. This position advocates for individuals, facilitates recovery-focused skill assessments, and connects clients with educational, vocational, and specialized healthcare services, including mental health, chronic conditions, and justice-involved support.
Work Schedule:
Monday, Wednesday, Friday, 9:00 a.m. to 5:00 p.m., and Tuesday and Friday, 11:00 a.m. to 7:00 p.m. Primary Duties and Responsibilities of a
Care Manager:
Provide direct support services to clients and their natural supports including psychoeducation, advocacy, crisis intervention, community integration, and skill development. Assist the team in coordinating care with other service providers in the Agency or community. Provide a comprehensive care management assessment that addresses life domains such as housing, finances, transportation, legal services, vocational, employment, healthcare and family strengths and needs. Participate as a member of the interdisciplinary team to ensure provision of services and referrals and linkage related to social determinants of health. Provides care management services to ensure that clients are both informed about and have the ability to access a full range of medical and treatment opportunities. Identifies priorities, desired outcomes, and the strategies and resources to be used for obtaining outcomes based on the care management assessment. Ability to educate and link to appropriate care resources in the community including resources that address specialized needs such as HIV/AIDS, Hepatitis C, mental health disorders, chronic and acute health problems, and problems stemming from involvement with the criminal justice system. Promotes recovery to the client's fullest potential by facilitating assessments of skills, including literacy, educational opportunities, job readiness skills, vocational training, employment placement and retention and career development. Serves as an advocate on behalf of the individual. Education, Licensure and Qualifications for a
Care Manager:
Bachelor's degree in social work or counseling, required. Minimum 2 years of experience in addictions and/or mental health field. NJ driver's license, required. Strong interpersonal skills and ability to build relationships across diverse backgrounds. Knowledge of community resources and ability to provide appropriate referrals. Experience with evidence-based practices and interdisciplinary teamwork. Commitment to confidentiality and ethical standards.
