Are you passionate about mental health and helping children and families succeed? Tri County Care Management Organization (TCCMO) is a non-profit organization that provides care management (case management) services for children with complex challenges in Hunterdon, Somerset and Warren Counties. We are currently seeking a Care Manager Specialist based in our Branchburg, NJ office to support care management activities while learning and practicing the core values of the wraparound process. This is a full-time position. Our Care Manager Specialists play a pivotal role, assisting the Operations Department in guiding youth and their families in the right direction to meet their goals and thrive by providing intensive wraparound services that help youth succeed at home, in school, and in the community. Responsibilities of a
Care Manager Specialist:
Assist Care Managers who provide intensive case management services to families and children with behavioral, emotional, social, developmental, and mental health challenges Provide transportation to youth and families Act as primary backup for Care Managers during absences Travel to various meetings at homes, schools, courts, and attend other services with the youth and families as needed Connect families to therapeutic and community services Complete progress notes, service plans, and other required documentation in electronic records according to agency standards Assist other agency departments as needed
Position Requirements:
Bachelor's degree in social work, psychology or related discipline Strong interpersonal and organizational skills Bilingual skills desirable
