POSITION SUMMARY
The Care Manager Supervisor will ensure that Health Home enrollees receive needed medical, behavioral, and social services with the goal of improving individual health outcomes. Will have overall responsibility and accountability for coordinating all aspects of the individual's care. Will collaborate with multidisciplinary personnel (i.e. medical, mental health, nurses, etc.) to assure patients receive services as indicated in the enrollee's plan of care. Responsible for supervising the CMA team members and ensuring patient centered efficient and effective operations. Participates in recruitment and selection process. Orient and train new staff. Complete Time & Attendance responsibilities. Coach, counsel, and provide performance feedback.
EDUCATION AND EXPERIENCE
Master's degree and two years of supervisory experience in a behavioral health setting or an applicable licensed level healthcare professional with prior experience in a behavioral health setting required. Three years of experience or two years of care management experience required. Healthcare education, certification, and/or training preferred. EMR Super User preferred.
PHYSICAL REQUIREMENTS
While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle objects; controls, reach with hands and arms; climb stairs; balance; stoop; kneel; talk or hear; taste or smell. The employee must occasionally lift up to 15 pounds. Specific vision abilities required by the job include close vision, distance and color vision, peripheral vision, depth perception and the ability to adjust focus.
