Part-Time Care Advocate / Aging Life Care Professional 15-25 hours per week, W2 Employee Make a meaningful difference without working nights, weekends, or a traditional 40-hour schedule. Abridge Aging Life Care Management is seeking an experienced Aging Life Care Manager / Care Manager to join our small, highly specialized team serving older adults and their families throughout the Athens and Greensboro Georgia areas. This is an excellent opportunity for a seasoned aging-services professional who enjoys problem-solving, building trusted relationships, advocating for clients, and helping families navigate complex care situations. You won't be working from a script or simply completing tasks. Our Care Managers use professional judgment to assess needs, coordinate services, communicate with families and providers, and help clients remain safe and well-supported in the setting that is right for them. What You'll Do Conduct client assessments and ongoing care management Develop and monitor individualized care plans Coordinate medical, social, home care and community services Communicate with families, physicians, caregivers and other professionals Advocate for clients and help resolve complex care challenges Make home and community visits with clients Document services and communicate updates to the care team Respond thoughtfully to changing client needs and family concerns Participate in care conferences and professional collaboration We're Looking For Someone Who Is Experienced working with older adults and complex care situations A strong critical thinker and problem solver Warm, compassionate and emotionally intelligent Comfortable working independently while being part of a small team An excellent communicator, both verbally and in writing Organized and dependable Comfortable navigating dementia, chronic illness, family dynamics and changing care needs Professional and confident interacting with physicians, attorneys, facilities, caregivers and families Preferred Background We welcome professionals with backgrounds such as: Aging Life Care / geriatric care management Social work Nursing Case management Gerontology Hospice or palliative care Home health Senior living / memory care Other closely related aging-services fields A bachelor's degree is required; advanced education and relevant professional credentials are strongly valued.
