As a Care Manager , you will play a vital role in coordinating services, supporting members through life transitions, and helping individuals reach their highest level of independence. What You'll Do Complete initial and ongoing assessments to develop person-centered care plans. Serve as the primary point of contact for members, ensuring consistent communication between caregivers, providers, and community resources. Facilitate smooth care transitions, including hospital discharges, incarceration releases, and other life changes. Lead interdisciplinary team meetings and coordinate with providers, discharge planners, social workers, and caregivers. Provide education, advocacy, and support to encourage member independence. Maintain accurate and timely documentation and reports. Deliver on-call coverage as part of a flexible work schedule. Provide transportation assistance for members when needed. What We're Looking For We seek a motivated professional who is passionate about whole-person care, thrives in a collaborative environment, and demonstrates strong organizational skills. Education & Licensure (Required): Bachelor's degree in psychology, sociology, social work, or a related human services field Experience (Required): Minimum of 2 years' experience working directly with individuals with behavioral health disorders
Preferred Skills & Abilities:
Strong understanding of care management and person-centered planning principles Ability to apply evidence-based clinical guidelines and coverage limitations Experience supporting individuals with chronic physical or behavioral illnesses across diverse populations Comfortable conducting home visits and community-based work Bi/multilingual abilities a plus Requirements Valid driver's license, clean driving record, and active auto insurance. Ability to lift/move up to 25-50 lbs. occasionally and travel as needed. Must maintain required certifications, including First Aid/CPR, defensive driving, and confidentiality/ethics training.
