In this role, you will provide Enhanced Care Management (ECM) and care coordination services to Medi-Cal members, helping individuals overcome barriers to stability, health, and wellness. We are looking for someone who is passionate about serving vulnerable populations, particularly justice-involved individuals and those experiencing or at risk of homelessness. About the Population We Serve This position requires experience working with the justice-involved population . Justice-involved lived experience refers to personal experience navigating the legal or criminal justice system, including direct interactions with courts, corrections, probation, parole, or related systems. Candidates with professional or lived experience supporting justice-involved individuals are encouraged to apply. Qualifications Bachelor's degree (BA/BS) in Social Work, Psychology, Human Services, or a related social science field required Minimum of two years of experience in a social services, case management, or care coordination setting Experience serving justice-involved individuals required Bilingual English/Spanish preferred Valid California Driver's License with a clean driving record Ability to pass: Department of Justice (DOJ) Clearance Federal Bureau of Investigation (FBI) Clearance Child Abuse Central Index Clearance (CAIC) Health Screening and TB Clearance Essential Skills and Abilities Understanding of human growth, development, and trauma-informed care Interest in serving individuals and families experiencing homelessness or housing instability Ability to maintain a respectful, objective, and non-discriminatory approach Strong interpersonal, engagement, and advocacy skills Excellent verbal and written communication abilities Professional, mature, and responsible work ethic Friendly, outgoing, and community-oriented personality Strong organizational and documentation skills Primary Responsibilities Care Coordination & Case Management Provide consultation, care coordination, and case management services to ECM participants. Collaborate with clients, family members, natural supports, and community partners to develop individualized care plans. Assist clients in establishing SMART goals and support progress toward achieving those goals. Monitor care plans, evaluate progress, and revise services as needed. Member Engagement & Support Maintain regular contact with assigned clients, including weekly outreach and monthly face-to-face visits, or more frequently when necessary. Transport participants when appropriate and assist with access to community resources and services. Manage a caseload of approximately 25 members, with the potential to support up to 50 members based on program needs. Collaboration & Advocacy Coordinate with medical providers, behavioral health professionals, social service agencies, housing providers, and other community partners. Maintain ongoing communication with multidisciplinary treatment teams to ensure comprehensive support. Documentation & Compliance Complete timely progress notes, case notes, quarterly reports, and other required program documentation. Administer the Adult Needs and Strengths Assessment (ANSA) at intake and every six months thereafter. Develop transition and discharge plans, including aftercare and follow-up support. Monitor housing placements to ensure compliance with applicable regulations and standards. Follow all State and Federal regulations regarding confidentiality, mandated reporting, and client rights. Program Operations Report any potentially harmful, illegal, or safety-related concerns to management immediately. Participate in on-call responsibilities as required to meet client needs.
