GENERAL PURPOSE
Leads by example through promoting and developing vibrant, purposeful and engaging resident programs for individuals living with dementia. Leads others to develop a well-trained compassionate Memory Care team that promotes dignity and choice for individuals living with dementia. Leads by example through implementation of engagement opportunities that align with successful aging in the areas of vital brains, vital bodies and vital connections.
RESPONSIBILITIES
Maintain basic knowledge of computer software and internet platforms. Develop programming strategy to ensure purposeful, successful and dignified engagement of all residents. Facilitate portions of daily programming to ensure positive engagement and model excellence in engagement for the team. Develop monthly engagement calendar that meets Vitality expectations and standards. Collaborate with Engagement Director on joint programming opportunities and shared resources to enhance overall experience for all residents and families. Develop relationships with cultural, artistic, intergenerational and other groups in the greater community that can provide resources to enhance engagement program. Collaborate with Wellness team in the development of resident care plans providing input based on knowledge of resident as well as incorporating the life journal information. Identify trends and changes in areas such as falls: unmet needs and other incidents, and work with interdisciplinary team (including Wellness Director, resident assistants, etc.) to develop plans and interventions to address trends. Lead interdisciplinary team meetings, when necessary, to analyze unmet resident needs and develop interventions and plans to effectively meet residents' needs through engagement, environmental changes and other non-pharmacological interventions. Coordinate and lead regular communication amongst memory care team. Direct, coach and mentor memory care team members in engaging residents, communicating with residents and overall performance in memory care daily routine of care and programming. Participate in Vital Connections process and meetings to ensure successful move-in experience for all residents and families. Collaborate with family members and resident in the initial completion and ongoing development of resident life journal. Work in partnership with Wellness Director in the development of transition plans for residents transitioning from Assisted Living to Memory Care. Establish strong relationships with families through consistent, ongoing and proactive communication. Establish, promote and maintain regularly scheduled family education opportunities on topics related to aging, dementia, caregiving, grief, etc. Collaborate with Sales Director in the discovery, tour and creative follow-up for families and residents considering memory care. Establish, promote and maintain a family networking group in which families are able to build relationships with one another independent of their loved one living with dementia. Ensure compliance with all federal and state or provincial regulations pertaining to health services. Attend and participate in onboarding, annual and in-service training and team member meetings, as requested. Respect dignity and confidentiality standards and promote resident rights. Complete or assist with special projects as requested by community or senior management. Adheres to all policies and procedures of the company. Perform other duties as assigned by the Executive Director.
LEADERSHIP FUNCTIONS
Participate in the Leader on Duty program. Conduct community tours to prospective residents and their families. Attend various community events. Exemplify Vitality leadership values during interactions with families, residents and team members. Responsible for the consistent execution of Vitality memory care standards to ensure community promotes dignity, success and vibrant lifestyle within the memory care environment. Supervise memory care team members including Engagement Director and Personal Care Specialists in conjunction with Wellness Director, which includes giving direction, coaching, providing consistent ongoing feedback and completing regular performance reviews. Provide education, coaching and overall development to all community team members to enhance their knowledge and skills working with individuals living with dementia and their family members. Effectively collaborate with community leadership team in order to coordinate consistent delivery of services such as dining, housekeeping, etc. within memory care environment. Increase team member recognition and reward through performance reviews and promotion. Attend and participate in training, in-services, and team member meetings, as requested. Understand and can implement all emergency plans including fire, hurricane, earthquake and other disasters. Assists in monthly training for team members and yearly evacuation plans for team members and residents. Desire to work with older adults and their families. Must possess excellent organizational skills and multi-tasking abilities. Project a positive and professional image at all times. Demonstrate experience in leading a team caring for older adults.
MANAGEMENT/DECISION MAKING
Applies existing guidelines and procedures to make varied decisions within a department. Uses sound judgment and experience to solve moderately complex problems based on precedent, example, reasonableness or a combination of these. SKILLS and
QUALIFICATIONS
Certified and licensed in Nursing or Social Work. Maintain current CPR certification. 5 years of experience working with and developing successful programs for individuals living with Alzheimer's disease, and related dementia. 2-3 years of experience as a manager with exceptional leadership skills. Ability to speak effectively before groups of customers or team members of the organization. Substantial knowledge of Alzheimer's disease and related dementia causing diseases and disease progression. Ability to develop dignified, purposeful and individualized programming to meet the needs of residents, families and team members. Must possess excellent organizational skills and multi-tasking abilities. Experience with developing, coaching, and leading others.
PHYSICAL REQUIREMENTS
Physical Demands include: Sitting, Standing, Walking, Bending, Reaching, Pushing, Pulling, Climbing Stairs, Grasping, Manual Dexterity, Vision Acuity, Tasting, Smelling, and Feeling. The weight determined to be appropriate for an Memory Care Director to lift with or without reasonable accommodations is up to 50 lbs.
