Job Description

Experteer Overview In this role you support care management and coordination for high-need members, working with a multidisciplinary team to deliver integrated LTSS and behavioral health services. You help ensure progress toward outcomes, contribute to cost-effective care, and coordinate waiver processes and care plans. You will liaise with members, caregivers, and providers to improve continuity of care, while navigating diverse settings. This position offers meaningful impact in improving member outcomes and service integration.

Compensation / Benefits
  • Complete comprehensive member assessments within regulated timelines, including in-person home visits
  • Facilitate waiver enrollment/disenrollment processes
  • Develop and implement care plans with members and care network
  • Monitor and adjust care plans based on effectiveness and milestones
  • Promote integration of LTSS, behavioral health, and community resources
  • Assess medical necessity and authorize waiver services
  • Evaluate benefits and funding sources
  • Lead interdisciplinary care team meetings for service approvals/denials
  • Use motivational interviewing and clinical guidelines to support change
  • Identify barriers to care and address psychosocial, financial, and medical obstacles
  • Identify critical incidents and develop prevention plans
  • Collaborate with licensed care managers/leadership as needed
  • Travel 25-40% depending on requirementsTasks
  • At least 2 years healthcare experience with LTSS exposure and care management experience
  • LPN or LVN licensure (active/unrestricted as required by state)
  • Bachelor's degree in a health-care related field may be required in some states
  • Valid driver's license and reliable transportation for travel
  • Knowledge of community resources
  • Ability to work across diverse settings and with diverse populations
  • Strong communication, problem-solving, and time-management skills
  • Proficiency in Microsoft Office and navigating online portals/databasesKey requirements
