RN, Wellness Director at The Courtyard at Fitchburg Assisted Living Courtyard at Fitchburg Fitchburg, WI Job Details Full-time $105,000 - $115,000 a year 1 day ago Benefits Health insurance Dental insurance 401(k) Paid time off Employee assistance program Vision insurance Qualifications Nursing Teamwork Long term care experience RN License Patient assessment Full Job Description The Courtyard at Fitchburg Assisted Living is looking for an experienced long term care Registered Nurse Wellness Director to oversee the clinical care for seniors who reside at our community. The RN Wellness Director is a key component to providing care and services to the residents in a home-like environment in which they receive individualized services based on their needs and preferences. The Registered Nurse assists with the medical components related to their care and services. Job duties include but not limited to: Assessments Care Planning (Development and Meetings) Coordinating Care and collaboration amongst internal teams, physicians, and third party providers Policy development and implementation Employee education and training Oversight of medication management program Quality assurance Completing medication reviews Resident education Must be a licensed nurse in the State of WI On call for staff to report and triage (rotates on weekends)
