The Director of Health and Wellness directs the Assisted Living Program, including the planning, development and overall operation of the Health and Wellness Department, which ensures Guests receive quality care 24 hours a day. Ensures the standards and state requirements for resident care are met. Directs staff, ensures regulatory compliance and is responsible for management of Assisted Living budget. Staff Development Attends and participates in scheduled in-service training, educational classes, and meetings to maintain current certification as applicable and managed by regulatory agencies and company policies. Participates in the QAPI program to review trends in the health and wellness department and develop process improvement plans. Leadership Fundamentals Ensures the Health and Wellness and ALF Department follows federal, state, and local regulations. Develop, implement, and update department policy and procedures when necessary or as directed. Oversee staffing to ensure Guest needs and regulatory requirements are met. Must answer and respond to call lights promptly and courteously when working in Guest care areas. Assist with new admissions, re-admissions and assist with the transfer of Guests to different rooms within the facility. Communicates and interacts effectively and tactfully with guests, visitors, families, peers, and supervisors. Attend and participate in departmental meetings and in-services. Provides leadership to the center in the absence of the Executive Director. Clinical Systems Works with residents, families, and other staff to ensure that residents are at appropriate levels of care. Monitors PCC to ensure that all regulatory times for review of assessments and service plans are met and that resident needs are met. Responsible for planning, directing, and supervising the activities of the nursing staff. Coordinates partnership and holds accountable external vendors i.e., hospice, home health, providers MD and NP, specialty doctors i.e., podiatrist, etc. Oversee and responsible for resident care services including Dementia Care Service to meet the needs of the resident on the Memory Care unit Oversees and responsible for resident care needs, i.e., Showers, Skin checks, monthly weights, and any concerns are addressed promptly. Oversee and responsible for partnership with pharmacy services, processing of new physician orders, communication with the pharmacy Coordinate the employee for tuberculosis testing, ensure it is conducted in a timely manner. Writes and reviews residents' assessments and service plans with appropriate staff. Maintains the comfort, privacy and dignity of Guests/guests and interacts with them in a manner that displays warmth, respect and promotes a caring environment. Personnel Development Schedules AND Conducts staff training programs and in-services for the Assisted Living staff. Schedules and conducts annual performance reviews of all subordinate staff. Provides guidance and education to staff on their performance. Conducts counseling and disciplinary action to subordinate staff members as needed. Responsible for training and educating staff members. Fiscal Management Manages labor costs, supplies and other line items in Sheltered Care SNF/Assisted Living/Memory Care budget. HPPD labor with budget, overtime with budget, no agency usage. Champion of census growth and timely assessment in the community of potential residents. Strive for census to meet goal. Administrative Responsibilities Directly oversees the delivery of care to Skilled Nursing Facility, Sheltered Care Assisted Living and Memory Care units to ensure that emotional, physical, psychological and safety needs are met through all program services and activities. Coordinates, along with the Executive Director, and family/resident care conferences and serve as a direct liaison between residents, families, and staff. Prepares and participates in Federal, State and Local agency annual surveys. Reviews any deficiencies noted by government agencies during inspections and directs implementation of changes needed to correct any deficiencies found. Ensures and monitors compliance with all regulatory times for review of PCC Forms. Completion and service plans for development are met, and that resident needs are met. Review report, follow through with RD quarterly recommendations. Coordinate with RD as necessary for resident care needs. Conducts assessments of potential residents for assisted living level of care. Conducts new resident orientation for all Assisted Living Residents. Ensure compliance with all state regulatory requirements including mandatory reporting to the State. Draft Initial and Final reports to the state as director by the ED and/or RDCO. Conducts investigations as necessary, obtains staff and resident statements, and follows all concern and incident reports to completion, filed and ensure that follow-up is conducted appropriately. Monitor and Ensure Incidents are completed in PCC. Coordinates the work of all nursing and other assisted living staff, communicating pertinent resident health and safety information. Performs any miscellaneous work assignments as required. Education Associate's degree in science, Nursing, or related field. Graduate from an accredited nursing program. Current State License as Registered Nurse or Licensed Practical Nurse. C.P.R. Certified Experience Five years in long-term or acute health care preferred; At least 2 years nursing supervisor experience required. Previous experience in a Nurse Leadership role. Physical Demands While performing the duties of this job, the employee is regularly required to talk and hear. This is a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets, and bend or stand, as necessary. Work Environment This job operates in a health care setting. This role requires regular walking to various locations around the facility. This role routinely uses standard office equipment such as computers, phones, photocopiers, and fax machines. This role may also encounter guests who may have contagious illnesses. Position Type and Expected Hours of Work This position is part of a health care facility that is open 24 hours a day, 365 days a year. Rotating shifts and holiday and work during inclement weather are required.
