Providing patient care including, but not limited to: passing medications, taking vitals, assisting with activities of daily living, and supervising CNA's. Assessing and reassessing patients throughout the shift and reporting patient concerns and changes in condition to the nurse supervisor and/or physician in a timely manner. Participating on the interdisciplinary team by providing the appropriate information to the team, and utilizing the information received to ensure positive patient outcomes. Collaborate with the interdisciplinary team to prepare and administer individualized nursing plans. Have responsibility for providing direct patient care to sub-acute patients and long term care residents, which may include those with Alzheimer's and Dementia. Assist the manager in supervising neighborhood staff. Maintain established safety, environmental, and infection control standards and takes appropriate corrective action for identified infractions. Accurately transcribe and implement physician orders seeking clarification if needed. Obtain accurate nursing history and documents on appropriate patient records. Implement prescribed treatment and nursing plan of care. Evaluate and revise plan of care as needed
