Position Summary The RN Director of Care Management leads the implementation, strategic development, and daily oversight of the Care Management, Utilization Management (UM), and Resource Management programs. This role ensures strict regulatory compliance, streamlines discharge planning, reduces avoidable days, and optimizes clinical outcomes through interdisciplinary collaboration. Core Responsibilities Care Management & Operations Oversee an effective, compliant Care Management program adhering to Joint Commission, federal, and state regulatory standards. Conduct chart reviews to evaluate Severity of Illness (SI) and Intensity of Service (IS) for admissions, continued stays, and discharge planning. Refer non-matching cases to the Physician Advisor or Utilization Management Committee for appropriate intervention. Identify avoidable days and implement proactive communication systems to resolve bottlenecks. Coordinate discharge planning and social services functions; act as liaison to community agencies for post-acute care placement. Active participation in Patient Care Conferences and Length of Stay (LOS) meetings. Ensure accurate reporting of CPS and APS cases and train staff on mandatory reporting laws. Perform other duties as assigned; maintain regular and reliable attendance and policy compliance. Leadership & Strategic Management Provide direct leadership, mentorship, performance evaluations, and scheduling for departmental staff within their defined scope of practice. Partner with executive leadership on strategic planning, budgeting, expenditure monitoring, and resource allocation. Evaluate and implement healthcare software and technology to drive operational efficiencies. Develop, update, and enforce departmental policies, procedures, and quality control protocols. Manage audit, inspection, and accreditation processes to maintain peak quality assurance. Maintain staff competencies and cover staff roles when operational needs require. Qualifications Education & Experience Requirements Bachelor's Degree in relevant field required OR Seven (7) plus years of direct experience in lieu of a Bachelor's degree required 3-5 years of experience in closely related field with Bachelor's degree required Master's Degree preferred 3-5 years of previous leadership experience preferred Licenses & Certifications RN
  • Registered Nurse
  • State Licensure and/or Compact State Licensure required BCLS
  • Basic Life Support within seven (7) days of hire or transfer to position required Knowledge, Skills & Abilities Expert knowledge of Utilization Review, Length of Stay (LOS) management, and healthcare regulatory compliance.
Strong interdisciplinary leadership, strategic planning, cross-functional management, and delegation skills. Advanced critical thinking, problem-solving, and time management capabilities. Proficiency with Microsoft Office, Google Workspace, EHR platforms, and healthcare data analysis tools. This position is not eligible for immigration sponsorship now or in the future.
