Job Description
AccentCare is seeking a Registered Nurse / Patient Care Manager Regional Floater for our home health team. In this role, you will travel across a defined region to deliver skilled nursing care, manage complex patient caseloads, and support local teams. You will assess patients, create and update care plans, coordinate with physicians and pharmacists, and ensure compliance with home health regulations. Working within a collaborative, innovation-focused culture, you will help drive high-quality, patient-centered care and mentor staff while gaining exposure to advanced pharmaceutical and clinical practices.
