Job Description
MLK Community Hospital seeks an RN Care Manager for our Emergency Department. This role coordinates care for diverse ER patients, ensuring safe transitions and optimal outcomes. Responsibilities include performing rapid assessments, developing care plans, collaborating with physicians and interdisciplinary teams, arranging admissions and discharges, and connecting patients with community and social resources. The RN Care Manager advocates for patients, supports families, promotes equity and culturally competent care, uses evidence based practices, and contributes to quality, safety, and process improvement initiatives in a fast paced setting. Responsibilities Coordinate and manage ER patient care from admission through discharge or transfer Perform rapid clinical and psychosocial assessments to guide care plans Collaborate with physicians and interdisciplinary teams to optimize care and utilization Plan and execute safe discharges, including follow-up appointments and referrals Connect patients with community and social service resources Educate patients and families on diagnoses, medications, and follow-up care Advocate for patient needs, equity, and culturally competent care Monitor quality metrics and participate in process and safety improvement initiatives Document care coordination activities in the EHR accurately and timely Comply with hospital policies, regulatory guidelines, and best practices Required Skills Emergency nursing Care coordination Discharge planning Clinical assessment Utilization management Electronic Health Records (EHR) Patient education Interdisciplinary collaboration Care planning Community resource navigation
