Job Description

MLK Community Hospital seeks an RN Care Manager for our Emergency Department night shift. In this role, you coordinate care for high-acuity patients, perform comprehensive assessments, develop and update care plans, and ensure safe, timely transitions of care. You will collaborate closely with ED physicians, social workers, and community partners to support complex medical and social needs while advocating for health equity. Responsibilities Provide comprehensive nursing assessment and ongoing care management for ER patients through the night shift Develop, implement, and update individualized care and discharge plans Coordinate transitions of care with inpatient units, post-acute providers, and community resources Collaborate with physicians, social workers, and ancillary teams to address complex medical and social needs Perform medication administration, monitoring, and patient education per ED protocols Ensure accurate, timely clinical documentation in the EHR and support utilization review processes Advocate for patient safety, equity, and culturally competent care for underserved populations Participate in quality improvement, throughput, and readmission reduction initiatives in the Emergency Department Required Skills Emergency nursing Care coordination Patient assessment Discharge planning Utilization review Electronic health records (EHR) IV therapy and medication administration Clinical documentation Interdisciplinary collaboration Patient and family education
