We are currently seeking an experienced, organized, and team-oriented Clinical Manager (RN) to join our Home Health team at our Burlingame, CA office . As a Clinical Manager, you will work closely with our Director of Patient Care Services (DPCS), Administrator, clinical staff, and interdisciplinary team to ensure patients receive safe, high-quality care while maintaining compliance with all applicable regulatory requirements. Position Overview The Clinical Manager (RN) is responsible for the supervision, coordination, and oversight of clinical services in accordance with physician orders, patient plans of care, agency policies, and applicable federal and California regulations. This position assists the Director of Patient Care Services (DPCS) and/or Administrator with clinical operations, staff supervision, quality assurance, documentation oversight, regulatory compliance, and care coordination. The Clinical Manager supervises clinical personnel including RNs, LVNs, PTs, OTs, SLPs, MSWs, and HHAs and plays an important leadership role in maintaining clinical quality and survey readiness. Responsibilities Clinical Supervision & Oversight Supervise and support clinical personnel in accordance with agency policies and regulatory requirements. Ensure patient care is provided according to physician orders and the individualized plan of care. Review and oversee Start of Care (SOC), Resumption of Care (ROC), recertifications, and discharges. Ensure plans of care are established, reviewed, and updated appropriately. Promote effective coordination of care between nursing, therapy, social work, home health aides, physicians, and other members of the interdisciplinary team. Regulatory Compliance Support compliance with Medicare Conditions of Participation (CoPs), California Title 22 Home Health regulations, and agency policies and procedures. Maintain ongoing clinical survey readiness. Assist with CDPH licensing surveys, accreditation reviews, and federal audits as applicable. Ensure clinical documentation is complete, accurate, timely, and compliant with regulatory standards. OASIS & Documentation Oversight Review OASIS assessments for accuracy, timeliness, and clinical appropriateness. Ensure documentation supports medical necessity and homebound status. Monitor Start of Care and other required documentation timelines. Ensure physician orders, including verbal and telephone orders, are obtained, processed, and returned within required timeframes. Identify documentation deficiencies and provide education or corrective guidance to clinical staff. Quality Assurance & Performance Improvement Participate actively in the agency's Quality Assessment and Performance Improvement (QAPI) program. Monitor clinical quality indicators including hospitalizations, infection control metrics, patient outcomes, and documentation trends. Conduct chart audits and clinical record reviews. Assist in developing and implementing corrective actions and performance improvement initiatives. Identify opportunities to improve patient safety, clinical quality, and operational efficiency. Infection Control & Patient Safety Support compliance with the agency's infection prevention and control program. Monitor adherence to PPE, hand hygiene, and infection control standards. Review patient incidents, adverse events, complaints, and other clinical concerns. Assist with investigations and implementation of corrective actions when necessary. Staff Competency & Development Ensure clinical employees maintain required competencies and qualifications. Participate in skills validations, annual competencies, and performance evaluations. Support orientation and training of new clinical employees. Identify educational and performance needs and coordinate additional training as appropriate. Serve as a clinical resource and mentor for field clinicians. Care Coordination & Communication Promote effective communication between physicians, clinical staff, patients, caregivers, and agency leadership. Ensure physician orders are obtained and implemented appropriately. Participate in case conferences and interdisciplinary team meetings. Assist clinicians with complex patient situations and changes in patient condition. Help resolve clinical concerns promptly and appropriately. Clinical Operations Support Assist with staffing and scheduling to ensure patient needs are appropriately covered. Monitor clinician productivity, visit utilization, and caseload distribution. Assist with assigning cases based on patient needs, geographic coverage, and clinician competency. Collaborate with agency leadership on clinical operations and staffing needs. Participate in the on-call rotation as required. Requirements Current, active, and unrestricted California Registered Nurse (RN) license
  • Required Minimum 2 years of Home Health experience
  • Required Supervisory, Clinical Manager, Case Manager, or leadership experience
