Job Description
Job Summary We are hiring an RN Care Coordinator for a 13-week contract in Chico, CA . The position focuses on coordinating patient care throughout the episode of care, utilization management, discharge planning, and collaboration with patients, families, physicians, multidisciplinary teams, and payors. The facility uses EPIC as its EMR. Responsibilities Coordinate care and services for assigned patients. Perform patient assessments, care planning, interventions, and evaluations. Collaborate with patients, families, physicians, care teams, and payors. Review admission/discharge criteria, observation stays, and readmissions. Evaluate quality and utilization cases. Promote appropriate utilization of healthcare resources. Assist with discharge planning and appropriate level-of-care placement. Advocate for patients and families throughout the care process. Facilitate communication among providers and multidisciplinary teams. May be assigned to the Emergency Department. Requirements Active California RN License Minimum 1 year of recent Case Management experience in an acute-care setting Current BLS/CPR certification Minimum 1 year of nursing experience EPIC experience preferred
