Part-time positionMonday Wednesday and Friday Qualifications :
1- A current CA-licensed RN. 2
- A Baccalaureate or higher degree in nursing. 3
- Three (3) years of experience within the last five (5) years in (A Home Healthcare Agency, primary care clinic or health facility). 4
- Strong knowledge of Medicare Home Health requirements, California regulations, OASIS, Plans of Care, and clinical documentation standards. 5
- Ability to effectively supervise multidisciplinary staff and manage multiple priorities in a fast-paced Home Health environment. 6
- Demonstrated commitment to patient safety, quality outcomes, compliance, and compassionate patient-centered care.
Experience:
One (1 ) year experience with supervision of professional administrative and field personnel.Benefits:
Other bonus upon passing CHAP survey.Responsibilities:
- Perform care as defined within the scope of the state's nurse practice act
- Assists in the development of organizational goals. Recommends and administers agency policies and procedures
- Supervises, educates and coaches other nursing personnel/paraprofessional staff
- Assists with in-services and training programs
- Assists with maintenance of client clinical records, statistics, and reporting requirements
- Perform SOC/evaluation/recertification visits and regularly re-evaluates the patient's nursing needs.
- Prepares clinical and progress notes and summaries of care
- Ensure compliance with Medicare, HIPAA, California Home Health regulations, and agency policies
- Provide clinical supervision, guidance, and mentorship to nursing and interdisciplinary staff, promoting safe, high-quality, patient-centered care.
