You will lead a dedicated team of nurses and healthcare professionals, coordinate care plans, and ensure compliance with all regulatory standards. Duties Lead and supervise a team of registered nurses and healthcare staff delivering home health services, fostering a collaborative and motivated work environment Develop, implement, and evaluate individualized care plans aligned with patient needs and clinical best practices Coordinate patient assessments, vital signs monitoring, and medical documentation to ensure accurate records and seamless communication among care teams Oversee compliance with HIPAA regulations, state healthcare laws, and long-term care regulations to maintain the highest standards of privacy and safety Manage utilization review processes, including
ICD-9/ICD-10
coding, DRG (Diagnosis-Related Group) management, and utilization management strategies to optimize patient outcomes and resource use Ensure proper medication administration, IV infusion procedures, catheter care, tube feeding, ventilator management, and sterile processing protocols are followed diligently Utilize electronic health record (EHR) systems such as Alora for documentation, scheduling, and reporting purposes Conduct regular team meetings to communicate updates on policies like nursing policy compliance and CHAPS standards while promoting professional development Monitor patient care quality through physical examinations, medical assessments, infection control practices, and diagnostic evaluations to uphold excellence in service delivery Have a strong QAPI and CMS knowledge Experience Proven leadership experience managing nursing teams within home health or acute care settings; supervisory experience preferred Extensive clinical background with critical care experience including Home Health Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and diagnostic evaluation processes Hands-on experience with EMR/EHR systems such as Alora; proficiency in medical documentation is essential Familiarity with long-term care regulations for Home health Agency; understanding of managed care policies Knowledge of infection control protocols and sterile techniques to prevent hospital-acquired infections across all patient interactions Excellent managerial communication skills for supervising staff performance while ensuring compliance with HIPAA privacy rules and safety standards Join us if you're ready to lead a passionate team dedicated to delivering exceptional home health nursing!
