You will be responsible for:

Leading interdisciplinary clinical and operational programs Managing staff performance and improvement initiatives Ensuring regulatory and accreditation readiness Overseeing case management, care coordination, and utilization management Facilitating discharge planning and patient progression Requirements for this role include: BSN required; master's degree in nursing, Health Administration, or Business Administration preferred Case Management certification (e.g., CCM or ACM) preferred Minimum five (5) years of experience in an acute healthcare setting required Minimum five (5) years of leadership/administrative experience in relevant fields required Advanced knowledge of case management and care coordination in an acute-care environment Apply today and let Capstone Health help you reach your full potential!
