Job Description
Under the supervision of the Multipurpose Senior Services Program (MSSP) Site Director/Supervising Care Manager, the Registered Nurse Care Manager - Casual (RNCM) functions as geriatric care manager for frail low-income elders enrolled in the MSSP Medi-Cal Waiver program. The RNCM evaluates the potential client as a total person and identifies the functional and health limitations that impede independent living. They recommend, provide, and coordinate a wide range of social and health services to assist MSSP participants in maintaining safe community living. The
