Job Description
Overview Nurse Next Door is a growing Home Health Agency serving the Greater Sacramento area. We are seeking an experienced, highly motivated, and proactive Registered Nurse to help strengthen our clinical operations, Medicare compliance, technology workflows, and survey readiness. This is an excellent opportunity for an experienced Home Health RN who enjoys working in a growing organization, solving problems, improving systems, and taking ownership of important projects. We are looking for someone who is comfortable being hands-on and understands that success in a growing Home Health Agency requires initiative, flexibility, responsiveness, and strong follow-through. About the Position The Home Health Clinical Operations Manager will play an important role in supporting clinical operations, regulatory compliance, documentation, quality assurance, survey readiness, and agency growth. The ideal candidate will have experience working in a Medicare-certified Home Health Agency and a strong understanding of Medicare Home Health regulations, Conditions of Participation, clinical documentation, OASIS, and quality assurance. Experience with the Axxess Home Health EHR system is highly preferred. We are particularly interested in an RN who is technically confident, resourceful, and comfortable learning and troubleshooting technology and clinical workflows independently . Key ResponsibilitiesClinical Operations & Compliance Provide clinical leadership and support to Home Health staff Review clinical documentation for completeness, accuracy, and compliance Ensure documentation meets Medicare Home Health requirements and agency standards Assist with OASIS review and documentation accuracy Review Plans of Care, physician orders, certifications, and recertifications Monitor documentation timelines and follow up promptly regarding incomplete or missing records Assist with patient admissions and ongoing episodes of care Support clinical Quality Assurance and Performance Improvement activities Help ensure compliance with Medicare Conditions of Participation and applicable regulations Assist with preparation for Medicare surveys, audits, and regulatory reviews Identify compliance gaps and assist in implementing corrective actions Axxess EHR & Technology Utilize and confidently navigate the Axxess Home Health EHR system Assist with optimizing clinical workflows within Axxess Help troubleshoot documentation and workflow issues Monitor documentation completion and follow up with clinicians Assist staff with navigating and utilizing Axxess effectively Work independently to identify technical or workflow issues and develop solutions Communicate with Axxess support when necessary Help develop efficient and compliant clinical workflows Survey Readiness Assist leadership with preparing the agency for Medicare certification and regulatory surveys Review clinical records for survey readiness Review personnel documentation and clinical competency requirements Assist with QAPI activities Assist with infection control and emergency preparedness requirements Identify deficiencies or gaps and help implement corrective action plans Participate in mock surveys and staff preparation Agency Growth & Referral Support Collaborate with leadership to support agency growth and development Assist in identifying patients who may benefit from skilled Home Health services Support outreach efforts by providing clinical expertise to referral sources Help educate referral partners regarding skilled Home Health services and qualifying needs Assist with identifying appropriate referrals for the agency, including patients who may qualify for limited pro bono services Build collaborative relationships with healthcare professionals and community partners Participate in agency growth initiatives as needed Leadership & Problem Solving Take ownership of assigned responsibilities and projects Identify problems proactively rather than waiting for direction Develop practical solutions and follow through until issues are resolved Provide guidance and support to clinicians Help hold staff accountable for documentation and compliance Assist with improving clinical systems and workflows Work independently while maintaining strong communication with agency leadership Demonstrate flexibility and responsiveness when clinical or operational needs arise Required Qualifications Current and unrestricted California Registered Nurse (RN) license - REQUIRED Minimum of 2 years of Home Health experience Experience working for a Medicare-certified Home Health Agency Strong understanding of Medicare Home Health regulations and documentation requirements Experience with clinical documentation and compliance Knowledge of Plans of Care, physician orders, certifications, and recertifications Strong organizational and communication skills Excellent attention to detail Strong problem-solving skills Ability to work independently and manage multiple priorities Highly responsive, dependable, and proactive work style Highly Preferred Qualifications Experience using Axxess Home Health EHR Experience troubleshooting or improving EHR workflows Experience with OASIS documentation and review Experience with Home Health Quality Assurance and Compliance Knowledge of Medicare Conditions of Participation Previous Home Health Clinical Manager, Clinical Supervisor, Director of Nursing, DPCC, or similar leadership experience Experience preparing an agency for a Medicare, state, or accreditation survey Experience with startup or growing Home Health Agencies Experience supervising, mentoring, or supporting clinical staff Experience with referral development or community healthcare outreach The Ideal Candidate The ideal candidate is someone who sees a problem and takes initiative to help solve it. Compensation & Schedule Pay:
$45.00-$55.00 per hour, depending on qualifications and experienceSchedule:
Part-Time Expected Hours:
Approximately 15-20 hours per week Hours and responsibilities may increase based on agency needs, growth, and candidate qualifications. This position offers an opportunity for professional growth and expanded clinical leadership responsibilities as the agency continues to develop.Pay:
