JOB SUMMARY
Oversees the direct and indirect care of Rocky Mountain Assisted Living residents by using a multidisciplinary approach. Ensures medication management, outside therapies and activities of daily living are being utilized to create an optimal level of care in a home life environment.
QUALIFICATIONS
Current LPN or RN licensure in Colorado, with at least one year experience. A desire to work with and a sympathetic attitude towards the elderly Ability to read and write in English and follow verbal and written instructions. Basic knowledge of Colorado State Health and Environment regulations pertaining to resident care. Experience working in a setting that provides care to the elderly. Ability to identify resident's physical, mental and psychosocial needs. Ability to work closely and professionally with Rocky Mountain Assisted Living staff, families, residents, outside agencies and vendors. Ability to remain patient, poised, and dignified in emergency or crisis situations. Knowledge in dementia care or ability to obtain training certification from Alzheimer's Association within 30 days of hire. Ability to maintain confidentiality of employee and resident information. Ability to sit or stand for prolonged periods of time. Ability to prioritize and multitask while maintaining a high level of organization. Basic computer skills, including the use of Word, Excel, Power Point, and Outlook.
RESPONSIBILITIES
Communicates professionally with Rocky Mountain Assisted Living residents, family members, staff, outside agencies and vendors. Assesses potential and returning residents in their current living environment, making sure the individual is assisted living appropriate based on company and accepted industry standards. Assists with move-ins with resident, families, physician, current placement (if any) and any other outside agencies. Communicates directly with move-in coordinator, ensuring new move-in paperwork is accurate and complete Completes resident care plan that meets the resident's mental, social, and psychosocial needs. Completes initial Medication Administration Record based on current and signed physicians order or medication list. Conducts in-services for Rocky Mountain Assisted Living staff on any topics pertaining with new move-in. Coordinates resident discharges with resident, families, physician, future placement (if any) and any other outside agencies. Cooperates with future placement ensuring requested paperwork is delivered in a timely and professional manner. Ensures Rocky Mountain Assisted Living homes are compliant with Colorado State Health Department regulations related to care and medication management. Maintains accurate and complete Medication Administration Records for each resident. Performs audits to ensure accuracy and completeness. Ensures physician orders are compliant with Rocky Mountain Assisted Living guidelines and the Colorado State Health and Environment regulations. Maintains accurate record-keeping in the homes. Ensures incident reports, changes in condition, fall risk assessments, and any other important information is well documented in resident progress notes and reflected on the care plan. Completes sufficient follow up on incident reports, medication issues and changes in resident condition. Assesses the need for preventative and restorative programs for residents. When appropriate, coordinates health services with residents, families, physicians, home health agencies, pharmacies, and other agencies and individuals. Assesses staff at each of the homes for educational needs and schedules in-services through outside agencies that cover pertinent topics associated with assessed needs. Takes corrective action and disciplines staff when a medication error or issue related to resident care has been identified. Retrains staff on identified issues whenever appropriate. Conducts medication pass audits of all QMAPs, making certain company policies related to medication administration and Colorado State Health and Environment regulations are being followed. Corrects and retrains QMAPs as necessary. Maintains proper documentation of narcotic's count and verification at each home.
