Job Overview We are seeking a dynamic and compassionate Licensed Practical Nurse (LPN) to step into the vital role of Charge Nurse within our healthcare team. In this energetic position, you will lead nursing staff, coordinate patient care, and ensure the delivery of high-quality clinical services across diverse settings such as long-term care, assisted living, or acute care units. Your expertise will help foster a positive environment where patient safety, comfort, and well-being are prioritized. As a Charge Nurse, you will serve as a clinical leader, advocate for residents and patients, and uphold compliance with healthcare standards and regulations. Duties Supervise and support nursing staff to deliver exceptional patient and resident care aligned with best practices and facility policies Coordinate daily clinical operations, including patient assessments, vital signs monitoring, and medication administration using EMR (Electronic Medical Records) systems such as Epic or Cerner Oversee patient care plans, ensuring accurate documentation of medical records, ICD-10 coding, and medical documentation in compliance with HIPAA regulations Assist with complex clinical procedures including IV infusion, catheterization, wound suturing, tube feeding, and sterile processing techniques Conduct comprehensive patient assessments utilizing physiology knowledge and perform diagnostic evaluations to inform treatment plans Manage emergency situations effectively by airway management, ventilator support, acute pain management, or trauma response in collaboration with hospital teams or Level I/II Trauma Centers Facilitate discharge planning and coordinate with case management teams to ensure smooth transitions for patients returning home or to other care facilities Provide education on health coaching topics such as dementia care, Alzheimer's care, pain management, or medication administration to patients and families Requirements Valid licensure as a Licensed Practical Nurse (LPN) in the state of practice Proven experience in critical care environments such as ICU or emergency medicine settings; hospital experience preferred Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks; familiarity with
ICD-9/ICD-10
coding is essential Strong background in diverse clinical areas including medical-surgical nursing, geriatrics, hospice care, infectious disease care, pediatric or neonatal ventilator management Experience working with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's or memory care units is highly desirable Knowledge of infection control protocols and aseptic techniques; ability to perform phlebotomy and venipuncture accurately Excellent patient assessment skills combined with anatomy knowledge and physiology expertise Ability to work effectively in fast-paced environments including urgent care or outpatient clinics; experience in trauma centers or hospital medicine is a plus Exceptional communication skills to collaborate across multidisciplinary teams while maintaining compliance with HIPAA standards Join us to make a meaningful difference in the lives of those we serve!
