81 Meriden Ave Bradley Memoria (10003) As part of a person-centered care model, this role will assist in management of daily operations of an interdisciplinary care team. This role will ensure the delivery of outcome based, cost-effective care, by utilizing metric-driven decisions. The primary purpose of this role is to oversee all patient care services and personnel for their assigned team. This role will collaborate with all team members to achieve exceptional outcomes.
JOB RESPONSIBILITIES
Assures the development of personnel qualifications and policies. Identifies and facilitates professional development needs and competency of staff through home visits with staff to supervise and evaluate clinical performance. Maintains compliance with regulatory agency policies and procedures. Accountable for team performance in achieving desired clinical and operational performance measures. Utilizes clinical software to identify opportunities for education, service utilization, patient progression, and team outcomes. Assures that clinical and functional outcomes are achieved at the highest level possible. Resolves patient care problems collaboratively with internal and external customers. Oversees daily patient assignments for clinical staff. Coordinates referrals and ensures timely start of care and service delivery. Ensures patient needs are continually assessed. Ensures individualized plans of care are developed, implemented, and updated. Ensures coordination of patient care and communication with physicians. Determines staffing needs and coordinates hiring. Hires, trains, coaches, and evaluates staff. Reviews and analyzes team and patient metrics. Participates in preceptor programs. Participates in performance improvement, huddles, IDT, and QAPI activities. Performs other duties as assigned.
Qualifications Minimum Requirements:
Associate Degree Registered Nurse Home Health or Hospice field experience Current Connecticut RN license
Preferred Requirements:
Bachelor's Degree in Nursing OASIS Certification Case Management Certification
