Conduct participant home visits, reassessments, and care evaluations. Monitor participant care plans and service delivery. Ensure adequate staffing and coverage for home care services. Coordinate care with contracted home care agencies and community providers. Participate in interdisciplinary team meetings and care planning. Oversee documentation, compliance, and quality assurance initiatives. Investigate and resolve participant concerns and service-related issues. Provide coaching, leadership, and performance oversight to field staff. Assist with policy development, process improvement, and program growth. Participate in on-call rotation responsibilities and respond to urgent participant care needs when necessary. Promote participant independence and quality of life through effective care management. Required Qualifications Required Active and unrestricted Washington, DC Registered Nurse (RN) License Minimum of two (2) years of Home Health, Home Care, Hospice, Community-Based Nursing, or Senior Care experience Experience supervising caregivers, HHAs, CNAs, LPNs, or nursing staff Strong clinical assessment and care coordination skills Experience serving elderly, frail, or medically complex populations Valid Driver's License Reliable transportation and current automobile insurance Ability to travel throughout Washington, DC for participant visits Strong verbal and written communication skills Proficiency with Microsoft Office and electronic documentation systems Ability to participate in on-call responsibilities when required Preferred Home Care Manager experience Clinical leadership experience Geriatric nursing experience Case management experience Program oversight experience Community-based healthcare experience Important Information This is a hands-on leadership position requiring regular travel throughout Washington, DC.
