Clinical Supervisor - Home Care Tailored Care Home Health Minimum 5 years of LPN experience required. Previous home care, home health, personal care, or similar supervisory experience strongly preferred. This is not an entry-level LPN position. This is not simply a nursing position. We are looking for an experienced LPN leader who is confident managing people, holding teams accountable, making sound clinical decisions within their scope of practice, and helping ensure our clients receive safe, consistent, high-quality care in their homes. The Clinical Supervisor will provide leadership and oversight across our caregiver/CNA workforce, scheduling team, field supervision, and clinical support functions . This individual will work closely with company leadership to strengthen communication, accountability, caregiver performance, client care, and day-to-day operations. What You'll Be Responsible ForClinical & Caregiver Leadership Provide leadership and appropriate clinical oversight for caregivers and CNAs providing care in clients' homes. Ensure caregivers are delivering care according to client care plans, agency standards, and applicable requirements. Support client assessments, reassessments, changes in condition, hospitalizations, and other care needs within the appropriate scope of practice. Identify concerns in the field and escalate or intervene appropriately when client safety, quality of care, or caregiver performance may be at risk. Support the team with complex client and caregiver situations. Help ensure accurate documentation and timely completion of required records. Maintain a strong presence and open communication with clients, families, caregivers, and staff. Scheduling & Operational Oversight Provide leadership and accountability to the scheduling team. Ensure client cases are appropriately staffed and scheduling concerns are addressed proactively. Work with scheduling to reduce open shifts, prevent gaps in care, and improve caregiver continuity. Identify staffing trends and operational barriers before they become larger problems. Ensure strong communication between scheduling, field staff, clinical support, and leadership. Hold the scheduling team accountable for follow-through, communication, documentation, and appropriate escalation of concerns. Field Team Supervision Direct and support the team member responsible for field supervision, caregiver evaluations, coaching, and training. Ensure caregiver field evaluations and competency expectations are completed consistently. Identify caregivers who need additional education, coaching, retraining, or corrective action. Help strengthen caregiver performance, professionalism, reliability, and retention. Ensure new caregivers are prepared to safely and confidently serve clients in the field. Leadership, Coaching & Accountability Lead with confidence, professionalism, consistency, and emotional intelligence. Establish clear expectations and hold team members accountable. Address performance and behavioral concerns promptly rather than allowing problems to continue. Conduct coaching conversations, performance improvement discussions, and disciplinary conversations when necessary. Partner with leadership on written warnings, final warnings, suspensions, and other corrective actions. Ensure employee performance concerns are documented appropriately and consistently. Help create a culture where excellent care, communication, reliability, teamwork, and professionalism are expected. Client Care & Quality Help ensure new clients transition successfully into services. Support assessments and reassessments within the appropriate scope of practice and agency requirements. Respond appropriately to changes in condition, incidents, hospitalizations, and client or family concerns. Work collaboratively with leadership and staff to resolve complaints and service concerns. Monitor quality trends and identify opportunities to improve client experience and continuity of care. Help ensure care plans remain appropriate as client needs change. Serve as a knowledgeable resource for caregivers, staff, clients, and families.
What We're Looking For Required:
Active and unrestricted Licensed Practical Nurse (LPN) license Minimum of 5 years of experience as an LPN Previous home care, home health, private duty, personal care, or community-based care experience Previous leadership, supervisory, or team-lead experience strongly preferred Experience working with and holding caregivers, CNAs, or other direct-care staff accountable Experience with employee coaching, performance management, corrective action, and difficult conversations Strong clinical judgment within LPN scope of practice and the ability to recognize when concerns require escalation Strong understanding of client safety, care planning, documentation, and quality standards Ability to lead both office-based and field-based team members Excellent communication, organization, and problem-solving skills Ability to manage competing priorities in a fast-moving home care environment Comfortable using technology and electronic documentation systems The Leader Who Will Be Successful Here We are looking for someone who takes ownership .
