Job Description
Drive Patient Progression Across Two Hospitals For an experienced Jacksonville-area leader, this opportunity offers a manageable regional relocation and increase in scope. Nealy Pierce has been exclusively engaged by a nonprofit 700+ bed dual campus health system to recruit a Director of Care Coordination (RN) . The Director will lead a 65-person department and own the strategies that move patients safely and efficiently through two hospital campuses. Key Priorities Improve length-of-stay performance Strengthen discharge planning and patient progression Resolve complex placement and bed-availability barriers Develop productive relationships with post-acute providers Align Case Management, Utilization Review, Social Work and Palliative Care Provide meaningful performance data to physicians and executives Support Already in Place The Director will inherit: Two experienced Case Management Managers A dedicated Utilization Review Manager An established, mature department A planned transition from a longtime leader Direct access to the Vice President of Medical Affairs Compensation and Benefits Up to $170,000 base salary, with flexibility for exceptional candidates Full relocation assistance 28 days of front-loaded PTO Executive-level benefits Candidate Requirements RN required Case Management or Care Coordination leadership within a large acute care hospital or system Five to seven years of acute care experience At least three years of leadership experience Master's required; BSN required if the master's is non-nursing CCM certification preferred Make a Strategic Move Up the Coast Remain in the coastal Southeast while gaining system-level responsibility and executive visibility.
