VitalCaring seeks a Clinical Care Manager, RN to coordinate and oversee home health and hospice services. In this role, you'll manage a caseload of patients, develop and update care plans, and ensure high-quality, evidence-based care is delivered in the home. You'll collaborate closely with physicians, therapists, social workers, and aides, guiding clinical decision-making and monitoring outcomes.
