Physician - Skilled Nursing Facility (SNFist) Position Summary We are seeking a compassionate and dependable physician to provide comprehensive medical care to patients residing in skilled nursing and post-acute rehabilitation facilities. The SNFist will manage medically complex patients, coordinate care with facility teams and specialists, support safe transitions from the hospital, and supervise assigned nurse practitioners. The ideal candidate is comfortable practicing independently in the post-acute setting, communicates effectively with patients and families, and values timely, high-quality, evidence-based care. Key Responsibilities Provide scheduled medical care at assigned skilled nursing facilities. Complete admission histories and physicals, routine follow-up visits, acute evaluations, and discharge assessments. Manage acute and chronic medical conditions, including post-hospital complications and changes in clinical status. Review hospital records, laboratory results, diagnostic studies, medications, and consultant recommendations. Develop and update individualized treatment plans based on each patient's clinical condition and goals of care. Participate in advance-care planning and goals-of-care discussions with patients, families, and facility staff. Coordinate care with nursing staff, therapists, specialists, hospice and palliative-care teams, social workers, and facility leadership. Respond appropriately to urgent clinical concerns and help determine when a patient requires emergency department evaluation or hospital transfer. Support safe transitions between hospitals, skilled nursing facilities, assisted living communities, and home. Complete accurate, compliant, and timely clinical documentation and coding. Participate in facility quality initiatives, including reducing avoidable hospital readmissions and improving care transitions. Maintain effective communication with patients, families, facility staff, and the organization's clinical leadership. Nurse Practitioner Supervision Provide clinical supervision and physician oversight for assigned nurse practitioners in accordance with organizational policies and applicable state requirements. Collaborate with nurse practitioners on patient management, treatment planning, and complex clinical decisions. Remain available for clinical consultation, escalation of care, and review of significant changes in patient condition. Review documentation, quality metrics, and clinical performance as required. Conduct regular clinical check-ins and provide ongoing coaching, feedback, and education. Promote consistent clinical standards and effective collaboration between physicians, nurse practitioners, nursing staff, and facility leadership. Complete required supervisory and collaborative-practice documentation. Qualifications MD or DO degree from an accredited medical school. Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatric Medicine. Active and unrestricted state medical license. Current DEA registration. Current BLS certification. Eligible for credentialing with participating facilities and insurance plans. Experience in skilled nursing, post-acute care, geriatrics, hospital medicine, or primary care is preferred. Prior experience supervising or collaborating with nurse practitioners is preferred. Strong clinical judgment, communication skills, professionalism, and time-management ability. Preferred Attributes Comfortable managing medically complex and frail older adults. Familiarity with Medicare requirements, SNF documentation, and evaluation and management coding. Ability to work independently while collaborating with an interdisciplinary team. Commitment to timely communication, continuity of care, and reducing preventable hospitalizations. Pay:
$220,000.00 - $350,000.00 per year
