Transitions Care seeks a Director of Clinical Operations RN / Hospice to lead hospice clinical services and ensure exceptional, compassionate end-of-life care. In this leadership role, you'll oversee daily operations, staff performance, and regulatory compliance while promoting a mission-driven, patient- and family-centered culture. You will guide interdisciplinary teams, optimize care coordination, manage quality and performance metrics, and support staff development.
