This field-based position works with patients, families, nurses, and clinical leadership to support care coordination, clinical oversight, staff development, and quality patient care throughout Northern Illinois. A company vehicle is provided for work-related travel . Responsibilities Provide clinical oversight and care coordination for assigned patients. Complete assessments, admissions, supervisory visits, and other clinical visits as needed. Support and maintain strong relationships with patients, families, and field nurses. Assist with nurse orientation, education, training, and competencies. Monitor clinical documentation and plans of care. Collaborate with clinical leadership and other members of the care team. Support quality, compliance, and continuity of care. Perform other clinical and administrative responsibilities as assigned. Qualifications Active Illinois RN license required. Pediatric, Private Duty Nursing, Home Nursing, Home Health, or care management experience preferred, but not required . Strong communication, organization, and relationship-building skills. Ability to work independently and manage multiple priorities. Comfortable with a field-based position and regional travel . Valid driver's license and acceptable driving record required.
