This role offers a dynamic balance between strategic clinical management in the office and direct, hands-on patient care in the field. As the Clinical Nurse Manager, you will serve as a vital bridge between administrative operations and field clinicians. You will oversee daily clinical operations, mentor field staff, ensure strict regulatory compliance, and step into the field to deliver exceptional direct patient care as needed. Key ResponsibilitiesClinical Leadership & Office Management Oversee daily clinical operations, clinical scheduling, and interdisciplinary team (IDT) coordination. Conduct regular clinical record reviews, audits, and oversight to ensure full compliance with CMS Conditions of Participation (CoPs), state regulations, and accrediting bodies. Mentor, train, and support field RNs, LVNs/LPNs, and hospice aides, fostering a collaborative, supportive clinical culture. Lead or participate in weekly Interdisciplinary Group (IDG) meetings, care plan reviews, and recertification processes. Manage triage communications, acute patient needs, and physician order processing from the office. Direct Field Care & Support Conduct direct patient visits in home, facility, or inpatient settings as needed to support staff workload, cover high-acuity cases, or assist during surge periods. Perform initial admissions, comprehensive assessments, or supervisory visits during complex clinical scenarios. Evaluate field staff competency through field visits and joint clinical evaluations. Provide direct crisis intervention, symptom management, and end-of-life care support when required.
