Clinical Supervisor - Home Health CareLink Home Health | Schaumburg, IL Full-Time | RN Required The Clinical Manager is responsible for directing and coordinating day-to-day clinical operations, ensuring high-quality patient care, and maintaining regulatory compliance across all disciplines. This role requires an RN with a solid understanding of home health regulations, Medicare requirements, and accreditation standards to lead clinical staff and drive quality outcomes. Responsibilities Oversee and coordinate all clinical services, ensuring quality patient care delivery across all disciplines (SN, PT, OT, SLP, MSW, HHA) Supervise, mentor, and evaluate clinical staff including field nurses, therapists, and home health aides Review and approve OASIS assessments, Plans of Care (CMS-485), and clinical documentation for accuracy and regulatory compliance Manage patient caseload assignments, monitor visit utilization, and ensure timely completion of SOC, recertification, and discharge workflows Conduct case conferences, participate in interdisciplinary team meetings, and provide clinical guidance on complex patient cases Ensure compliance with physician order management, timely Face-to-Face encounters, and certification/recertification timelines Monitor patient outcomes, HHCAHPS scores, and quality metrics; implement corrective actions as needed Ensure clinical operations align with CMS Conditions of Participation (42 CFR Part 484), ACHC accreditation standards, and state regulations Participate in internal audits, survey preparation, and quality improvement initiatives Collaborate with the Compliance Officer and Administrator to address regulatory changes and maintain survey readiness Participate in on-call rotation to support after-hours clinical needs
