Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) serving as a Charge Nurse during the 12pm to 8pm shift with rotating weekends. Responsibilities Lead and supervise nursing staff during your shift, ensuring adherence to care plans and facility policies. Conduct comprehensive patient assessments, vital signs monitoring, and physical examinations to inform care decisions. Administer medications, including injections and IV infusions, following strict protocols and documentation standards. Coordinate interdisciplinary communication for discharge planning, case management, and specialized treatments such as Alzheimer's or dementia care. Oversee infection control practices, sterile techniques, and medication administration to maintain safety standards. Utilize EMR/EHR systems like Epic or Cerner to document patient information accurately and efficiently. Respond promptly to urgent needs such as acute pain management, triage assessments, or emergency situations including airway management or ventilator support. Qualifications Valid Licensed Practical Nurse (LPN) license in good standing with the state licensing board. Proven experience in long-term care facilities, skilled nursing facilities, or assisted living environments. Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10). Experience working with geriatric patients, individuals with developmental disabilities, or those requiring memory care such as Alzheimer's or dementia care. Competency in IV infusion therapy, medication administration, and basic life support (BLS). Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth; experience with medical documentation is preferred. Excellent communication skills for caregiver collaboration and patient assessment; ability to handle complex cases involving acute care or trauma center protocols is a plus.
