Duties Help Duties and Responsibilities :

Organizional/Administrive Role:
Interpret and transle SM limitions from non-military providers. Draft language in web-based system and submit recommendions for SMs with health limitions to profiling military provider for considerion and signure. Assists in ongoing pient care coordinion until planned outcomes are achieved. Unless otherwise directed by the DSS, support every weekend Medical Readiness Event (MRE) in the ste with approprie staff to schedule meetings with SM in case management, upde unit leadership, tend medical synchronizion meetings and may provide dental support. Uses automed systems to document and track a variety of da reled to case management and care. Integres personal da files with other available automed systems to provide informion required for briefings, presentions, reports. Maintain local mobilizion medical readiness informion dabases and spreadsheets to provide monthly stus reports to units and OSS. Each of these processes as dicted by applicable regulory, NGB or OSS guidance. 2.
Consultant/Collaboror Role:
Provides advice, guidance, and health teaching to SM and Chain of Command, in individual or group settings designed to assist the SM in understanding, accepting and following medical recommendions. The incumbent assists in coordining comprehensive health care services to support individual Medical Readiness requirements in support of Ste medical readiness. Presents, with minimal supervision, cases and summary informion to senior leaders such as unit commanders and Ste Surgeon Staff. Incumbent will hold quarterly reviews with members of the health care team (OSS) and chain of command. Provide guidance and interpretion of physical profile limitions to Unit POCs. Works with the Medical Standards Officer to insure the Soldier transitions an expected re through the proposed care/trement plan. Problem solves with the team, on an individual or system basis to correct problems, prevent negive outcomes, inefficiencies. Provides support, clinical expertise, and coaching to personnel within the Ste Surgeon's Office and Traditional Drilling Medical Providers within the organizion. Participes in a continuing process improvement program for ongoing systemic monitoring and evaluion of pient outcomes to include tracking approprie da. Works with personnel for processing of line of duty investigions IAW AR 600-8-4, medical boards for the disability evaluion system IAW AR 635-40 and DA PAM 635-40 and composing/processing requirements for Soldiers eligible for care under all programs for injured Soldiers which include but are not limited to RCMC, and
INCAP IAW
all written guidance and policy. 3.
Educor Role:
Coordines and implements health prevention and educion programs reled to fitness for duty, medical readiness, and deployability. This will encompass complex 1:1 interaction with SM due to the sensitive issues th require discretion yet have a profound impact on deployability. Facilites and coordines communicion and educion between service member and all relevant members of the health care team and chain of command to optimize medical readiness and improve coordinion with unit commanders on case management issues, community resources, or other issues. Educes the SM for care, approprie use of their medical facilities and services. Provides educion and guidance on applicable medical transition programs IAW current regulory and stutory guidance through interdisciplinary collaborion, determines the prioritizion, goals, and care plan. Maintains documention and dabase for outcome evaluion as required. 4.
Professional Demeanor:
Assumes responsibility and accountability for case management of all Soldiers in case load. Integres management and case management processes to provide a framework for safe, effective, ethical decision-making within each of the case management team's roles. Supports the organizional vision, mission, and goals. Displays deep rooted values in accord with those of the Army and encourages the same in peers, co-workers, and Soldiers. The incumbent shall have team meetings as required based on coordinion CM efforts with medical readiness NCO's, Health Systems Specialist Office, and the Ste Surgeon office. with the Deputy Ste Surgeon (DSS), the Ste Surgeon (SS), and the Ste Dental Officer (SDO) (or appointed representives) to review all cases in progress. The incumbent shall be responsible to know the following ARNG web-based electronic da systems to include but not limited to: Medical Operions Da System (MODS): Medical Non-Deployable (MND), Dental Classificion Module (DENCLASS), Health Readiness Record (HRR), Medical Da Protection System (MEDPROS), Soldiers Pient Locor (SPL), and the Warrior Transition Report (WTR). 5. GENERAL EXPERIENCE
: Must possess understanding of the principles and concepts defined by a practical knowledge of health services activities such as, administrion, managed care, human resource management, logistics management, pient administrion, budgetary and fiscal management, medical manpower, medical facility management, biometrics, medical recruiting, or aeromedical evacuion.
SPECIALIZED EXPERIENCE
: Must possess least one (1) year (12 months) of specialized experience equivalent to the next lower grade level (GS-09). This experience must demonstre knowledge of basic principles and practices reled to the management of occupional health care delivery systems; experience in planning for and executing ste occupional health programs; and least six months of experience in one of the medical service core functions (health facilities, health plan management, medical resources, health care informion, etc.). Must have demonstred skills in written and oral communicion. Must possess successful completion of a PhD or equivalent doctoral degree from a professional nursing educional program or reled medical science field; OR least one full year of professional nursing experience (equivalent to the GS-09 grade level) and possession of a diploma, associe degree, bachelor's degree, or master's degree from a professional nursing educional program.
