RN Clinical Care Manager

PrimeSTAR Home Health is seeking a Full-Time RN Clinical Care Manager to join our team of one of the fastest-growing Home Healths in Northeast Louisiana. This position serves patients in Rayville, LA and surrounding areas. This position is located in our Rayville, LA office.

We are currently seeking an energetic Registered Nurse/Clinical Manager to assist the agency with the coordination of home health services to patients in their place of residence in accordance with physician orders. The RN Clinical Care Manager is under the supervision of the Director of Nurses. The RN Clinical Care Manager is available all times during operating hours and participates in all activities relevant to professional services furnished, including the management of patient care. Must be able to manage agency activities with DON not present. The RN Clinical Care Manager will participate in developing standards which ensure safe and therapeutically effective service to patients and families. Ability to provide oversight in a positive manner to team members in order to meet the needs of the agency.

Required experience:

We are seeking Registered Nurses (RN) whose license is active and in good standing with the Louisiana Board of Nursing Excellent clinical nursing skills and a background in ICU, ER, med/surg, or related skilled nursing care Experience with Home Health nursing and Medicare / OASIS a plus Willingness & ability to drive throughout our territory Other duties as assigned
