This is a hands-on leadership position for an experienced home health RN who is comfortable working closely with nurses, reviewing clinical documentation, supporting patient admissions, monitoring quality, and helping build strong clinical processes in a growing agency. The Clinical Manager will work under the supervision of the Director of Nursing and will have meaningful ownership of day-to-day clinical operations. Key Responsibilities Oversee daily clinical operations for LPNs, contract nurses, and OASIS RNs. Perform in-person field evaluations and provide clinical mentorship to nursing staff. Educate and train nurses on clinical standards, documentation, Medicare requirements, and compliance. Conduct clinical chart audits, including OASIS reviews, for accuracy, completeness, and compliance. Coordinate with the Intake Team and

to ensure timely and appropriate patient admissions. Perform required 60-day RN supervisory visits in accordance with Medicare Conditions of Participation. Monitor clinical quality indicators, including falls, hospitalizations, and rehospitalizations. Identify clinical, documentation, and compliance concerns and implement corrective actions. Coordinate with the Operations Manager regarding authorization tracking and billing-related clinical documentation. Communicate with physicians, specialists, referral sources, patients, and families regarding clinical matters. Lead weekly clinical team huddles to review active patients, recertifications, discharges, and clinical concerns. Track referrals from receipt through admission and monitor applicable clinical and compliance metrics. Participate in annual performance evaluations for clinical staff. Recruit, interview, screen, and recommend qualified clinical candidates to the Director of Nursing. Assist with OASIS assessments and other clinical needs as patient volume requires. Review and order clinical supplies for field staff. Lead QAPI (Quality Assurance Performance Improvement) meetings and initiatives. Develop Plans of Correction and quality improvement projects as needed. Work with the compliance consultant on ongoing regulatory and survey readiness. Manage clinical incident investigations and participate in root cause analysis. Prepare and provide monthly clinical quality reports to the Director of Nursing.
Qualifications Required:
Current Massachusetts RN license in good standing. Minimum 3 years of home health experience . Strong understanding of home health clinical operations and documentation. Working knowledge of PDGM, Medicare Conditions of Participation, and MassHealth home health regulations . Strong written and verbal communication skills. Ability to perform occasional in-home visits within the agency's service area. Comfortable working in a fast-paced, growing home health environment.
Preferred:
Previous Clinical Manager, Assistant DON, Clinical Coordinator, or Nurse Supervisor experience. OASIS-E certification or willingness to obtain certification within 90 days.
