We are seeking a detail-oriented, organized, and compassionate LPN Clinical Coordinator to join our growing team. This position is ideal for an LPN with strong assessment and clinical skills who enjoys coordinating care, collaborating with interdisciplinary teams, and ensuring quality patient outcomes. While this is primarily an office-based Monday through Friday position, occasional home visit coverage will be required to support our patients and clinical team. This role is primarily on site in our Fall River office. Schedule Full-Time Monday - Friday Day Shift Occasional home visit coverage as needed Responsibilities Coordinate and support daily clinical operations. Review and maintain accurate clinical documentation. Assist with patient admissions, care coordination, and ongoing case management. Communicate with physicians, referral sources, patients, and families to ensure continuity of care. Monitor patient records for regulatory compliance and quality standards. Collaborate with nurses, therapists, social workers, and other members of the interdisciplinary team. Assist with scheduling, authorizations, and other clinical administrative tasks. Provide clinical support and guidance to CNA staff when needed, and clincial support to the interdisciplinary team Conduct occasional home visits to provide clinical coverage and patient assessments. Maintain compliance with all federal, state, and organizational regulations. Qualifications Current Licensed Practical Nurse (LPN) license in good standing. Strong clinical assessment and critical thinking skills. Previous experience in home health, hospice, long-term care, or case management preferred. Excellent organizational, documentation, and time management skills. Strong verbal and written communication abilities. Proficiency with electronic medical records (EMR) systems. Ability to prioritize multiple responsibilities in a fast-paced environment. Valid driver's license and reliable transportation for occasional home visits.
