Job Overview We are seeking a dynamic and compassionate Licensed Practical Nurse (LPN) to step into the vital role of Charge Nurse within our healthcare team. Duties Lead and supervise nursing staff to ensure efficient and quality patient care delivery across all shifts. Coordinate patient assessments, vital signs monitoring, and comprehensive physical examinations. Administer medications, injections, IV infusions, and perform catheterization with precision and adherence to protocols. Oversee medical documentation, including electronic health records (EHR/EHR systems), ICD coding (International Classification of Diseases), and HIPAA compliance. Manage complex care plans for residents with diverse needs such as dementia, Alzheimer's, memory care, hospice, and long-term care regulations. Conduct triage assessments for incoming patients or residents requiring urgent or emergent attention. Facilitate discharge planning, case management, and collaboration with multidisciplinary teams to optimize patient outcomes. Requirements Valid Licensed Practical Nurse (LPN) license in good standing. Proven experience in critical care settings such as ICU, emergency room, or trauma centers preferred; Level I & II trauma center experience highly valued. Strong background in geriatrics, long-term care facilities, skilled nursing facilities, or assisted living environments. Familiarity with EMR/EHR systems Demonstrated expertise in medication administration, IV infusion techniques, catheter care, and basic life support (BLS/CPR). Knowledge of medical terminology including
ICD-9/10
coding and diagnostic evaluation procedures. Experience working with individuals with disabilities or developmental delays; Alzheimer's and dementia care experience is a plus. Ability to perform physical examinations, specimen collection/processing, suturing, and infection control practices confidently. Excellent communication skills for caregiver interaction and patient assessment; ability to work effectively within a team environment.
